Registration form for organizations

This form allows you to register your organization to participate in our sailing activities. You must: 

1 - Register your organization and your representative (Social worker, nurse, companion, etc)

2 - You will receive a registration confirmation containing a link to register each participant to the activity. These participants will now be linked to your organization. 

3 - If your organization's representative has changed, please contact us at (438) 395-5362 to make the changes. 

AQVA Rules to Be Respected by Groups

1. Maximum Capacity
The total number of participants cannot surpass eight sailors and four staff unless special arrangements have been made with the AQVA manager.

2. Life Jacket Requirement
Sailors must put on a life jacket before going on the dock.

3. Dock Access
Sailors and their caregivers may not go on the dock unless they are accompanied by a member of the AQVA staff.

4. Sailor Profile
A mini-profile describing the abilities and needs of each sailor must be completed and sent to AQVA at least two weeks prior to the sailing date. AQVA reserves the right to cancel a sailing outing if the mini-profile has not been received 48 hours before the sailing date.

5. Authorization Form
All sailors (or their parents or legal guardians) must have signed the consent form and returned it to us before going sailing.
